Existing patients may request supplement refills via this form.
Please be as specific as possible with the brand, size and quantity you need and indicate if you prefer to pick up in the office of your choice or have your order shipped. We will call you for payment information and to confirm the order.
A flat shipping rate of $8 will be applied with free standard shipping on orders over $75. There is no additional charge for orders that are picked up.
